1.PAN Card

2.Address Proof (Adhar card / Voter ID / Passport / Bank statement with address / Electricity Bill / Govt Add Proof) - any one

Note: The name on address proof should be exactly same as that on PAN Card

If name match - Provide self declaration letter by bank / attesting authority

3.Income Proof (6 months bank statement / 1 month Salary Slip / latest IT return / latest Form 16) - any one

4.Cancelled Check - (Name should be printed on check)

Note: If name not printed - provide bank passbook first page photocopy (xerox) / bank statement with IFSC code & MICR code.
